5 Governance Mistakes Teams Make When Deploying AI Agents

AI governance is one of those topics that everyone agrees is important and almost nobody gets right on the first attempt. After working with dozens of teams deploying AI agents to production, we have seen the same mistakes come up again and again. Here are the five most common — and how to avoid them.

1. Treating Governance as a Post-Launch Checklist

The mistake. The team ships an AI agent to production, then asks legal and compliance to “review it.” Governance becomes a sign-off step at the end of the development cycle.

Why it matters. By the time the agent is in production, its architecture is fixed. Retrofitting PII detection, audit logging, or content filtering into an existing pipeline is expensive. Worse, the agent has already been processing real user data without guardrails, creating compliance exposure from day one.

The fix. Define governance policies before the agent ships. Use a proxy layer that enforces policies at the network level so guardrails are active from the first request. 2. Relying on Prompt Instructions for Safety

The mistake. The team adds “never reveal customer data” to the system prompt and considers the PII problem solved.

Why it matters. Prompt-level instructions are suggestions, not enforcement. LLMs can be coaxed into ignoring system prompts through carefully crafted inputs. Prompt injection attacks are well-documented and increasingly automated. A system prompt is not a security boundary.

The fix. Enforce safety at the infrastructure level. Use regex-based PII detection, NER models, and content classifiers that run before and after every LLM call. These layers operate independently of the model’s behavior, so they cannot be bypassed through prompt manipulation. Defense in depth — not defense in prompts.

3. No Audit Trail

The mistake. The team logs LLM requests to their application logs, but there is no structured, immutable record of what the agent was asked, what it responded, and which policies were applied.

Why it matters. When an incident occurs — a customer reports that the agent shared incorrect medical information, or an auditor asks for evidence of PII handling — the team has to reconstruct events from scattered application logs. This is slow, error-prone, and often incomplete. For regulated industries, the absence of a proper audit trail can be a compliance violation in itself.

The fix. Every request through the AI pipeline should produce a structured audit record that includes: the input, the output, the model used, the policies evaluated, the policy results, and a timestamp. 4. One-Size-Fits-All Policies

The mistake. The team writes a single set of governance rules and applies them uniformly to every agent, every endpoint, and every user.

Why it matters. Different agents handle different types of data and serve different risk profiles. A customer-facing chatbot handling medical queries needs stricter PII controls than an internal code-review assistant. Applying the strictest policy everywhere adds latency and false positives to low-risk workflows. Applying the loosest policy everywhere leaves high-risk workflows exposed.

The fix. Scope policies to specific agents, routes, or environments. Use a policy-as-code approach where each agent configuration references the policies that apply to it. This lets you enforce strict PII redaction on customer-facing agents while allowing more permissive rules for internal tooling. 5. Ignoring Cost as a Governance Concern

The mistake. The team treats AI cost management as a finance problem, not a governance problem. There are no per-agent budgets, no alerts on spend anomalies, and no attribution of costs to specific teams or customers.

Why it matters. Uncontrolled AI spend is a governance failure. A misconfigured agent that enters a retry loop can burn through thousands of dollars in minutes. A prompt that unnecessarily includes the entire conversation history inflates token counts and costs without improving quality. Without cost governance, there is no accountability and no early warning system.

The fix. Set per-agent and per-customer cost budgets with automated alerts. Track token usage at the request level and attribute costs to the team or product that owns the agent. Treat cost anomalies the same way you treat error rate spikes — as incidents that need investigation.
